Information & Assistance: Information about senior services and access to needed services. There is no charge for this service.

Senior Social Club, an Adult Day Program: Socialization and care for seniors in four clubs throughout Sonoma County.

Care Navigator: Home visits to assess needs and recommend services.

Senior Care Coordination: Long-term care coordination by bonded, insured and supervised professionals.

Senior Peer Support: Trained volunteers who help seniors struggling with serious mental illness to access services and programs.

Healthy Ideas: Depression screening, case management and referrals to appropriate professionals.

Family Justice Center: Elder advocacy and care coordination for victims of domestic violence and financial abuse.

Daily Money Management: Bonded and insured counselors who assist seniors in bill pay, checkbook reconciliation and other financial needs.

Conservatorships: Court-appointed fiduciaries for those unable to manage their financial affairs.

Trusts: Professional trust management services.

Estate Planning: Comprehensive legal services emphasizing estate planning and advance healthcare directives for individuals 50 and over.

Lawyer Referral Service: State Bar-certified referral service to a panel of elder law attorneys.

 

Nutritional Services
